                                                             GUARANTEE and CONTRACT

You have purchased a good quality puppy. Every effort has been made to ensure that this puppy is going home with you in good health.

WE REQUIRE THAT YOU BRING THIS CONTRACT WITH YOU SIGNED. WITHOUT THIS SIGNED CONTRACT THERE IS NO GUARENTEE. Thank you!

                                                                     We GUARANTEE:

1.The puppy has been seen by our vet (Dr.Kevin Carter 540-879-3383 or Dr.Burgess), and has had a physical exam, and has been found to be in good health.

2. The puppy has had his/her 1st set of shots at 6 wks of age.

3. The puppy has been wormed at 3 and 5 wks of age with Pyrantel Pamoate and at 7 wks of age with Panacur.

4. The puppy will remain free from genetic or heredity defects, causing death or necessary euthanasia for 1 year dating from birth. (FOR BREEDERS ONLY - Whom we have given full registration rights - we give an extra 6 mo on this contract - from the birth of this puppy purchased).

IF such a condition should surface, we will replace the puppy with a puppy of EQUAL value, as soon as possible. WE DO NOT REFUND MONEY.

To be eligible for a replacement, please read below.

                                                          FOR THIS GUARANTEE, YOU MUST:

1. Have your puppy up to date on all vaccinations. The vaccinations must be administered by a veterinarian.

2. Feed your puppy a good quality puppy food for large breeds, and provide adequate care and shelter. Your puppy shall never be tied or in a kennel 24/7.

3. Do not let your puppy go up and downstairs/jump off any high furniture or vehicles/over-active activities/take this puppy on mile runs till he/she is over 1 yr of age.

4. You MUST use our kennel name CHURCH MOUNTAIN in the beginning part of the registered name, you select through the AKC registration process, for your puppy.

Failure to do so, does VOID our contract.

5. FOR BREEDERS ONLY - That we have given full registration rights to. YOU MUST  have this puppy OFA’d(prelim) or Pinn-hip(Hips), have a CERF(eye) exam and SAS(heart clearances) done by 16 mo of age. FAILURE to do so , makes this contract null and void.

                                         

                                                            CONTRACT LIMITATIONS

1. Buyer has 3 days to have the puppy examined by a veterinarian.

IF the puppy is found to be in poor health(this does not include worms/coccidia/giardia/dehydrated) please have your veterinarian contact us immediately and our veterinarian.

The puppy may be returned for a replacement puppy, if available or an upcoming litter. No money will be refunded.

2. IF within 1 year dating from the birth of the puppy, a hereditary or congenital defect should surface, and the puppy dies or has to be euthanized, the puppy willl be replaced with another puppy of EQUAL value as soon as possible. NO MONEY REFUNDED..(We will replace your puppy with another puppy). (WE DO REQUIRE THAT ALL AKC papers be returned back to us, without these papers, your puppy will not be replaced and contract is null and void) Buyer understands that dogs are bred selectively, and that a puppy may not be available immediately.

3. Proof of said defect must be evidenced by a letter from a veterinarian. We will then ask you to bring your puppy to OUR veterinarian to be examined and to confirm the diagnosis(breeder’s expense).

4. (For breeders with full registration rights) Breeder makes NO GUARANTEE as to the ability or quality of this puppy to reproduce.

5. (For breeders with full registration rights) Once this dog has been bred, and guarantee is null and void

6. Guarantee available to ORIGINAL purchaser only! And is non-transferable.

7. IF the puppy is replaced with the frist 3 days, the replacement puppy will carry the original guarantee. In the event that a puppy must be replaced at a later date, the replacement puppy does not carry a guarantee beyond the 3 days after seeing your veterinarian.

THIS PUPPY  IS TO NEVER BE SOLD TO A BACKYARD BREEDER,,PUPPY MILL, PET STORE,  S.P.C.A, RESCUE ORGANIZATION or REHOMED without contacting us first.  WE REQUIRE THE RIGHT OF FIRST REFUSAL OF ANY  DOG THAT IS TO BE SOLD OR GIVEN AWAY.
