Please fill out this form and return with your $300.00 deposit, to reserve(or acquire) a puppy from Church Mountain Kennel.

Some questions, to get to know you and plans for your new family member.

Question 3:Would you like a light golden, golden, light red or dark red puppy? Or you can choose, does not matter.

Question 4:Do you plan to breed this puppy, in the future?

Question 5:Do you have a fenced in yard?

 Question 6:  Do you have children?

Question 7:  Do you have any other dogs?

Question 8:  Have you ever owned a golden retriever before?

Question 9:  Will this puppy be primary inside or outside?

Question 10:  Do you plan to spay/neuter this dog, if not breeding?

                       (WE DO REQUIRE IF NOT BREEDING, THAT YOU DO HAVE YOUR PUPPY SPAYED/NEUTERED BY THE AGE OF 1 YR).
